Our vision is to establish Residential Therapeutic Communities for young people aged 16-25 with drug/alcohol problems and other associated addictive behavioural issues.
The RTC bridges the gap between residential rehabilitation centres and community-based day programmes, with a number of services being delivered in the community while residents live together supported by the Transitions team.
These 3 words best describe what Transitions is all about – a Residential Therapeutic Community*, with a difference.
Residential
Accommodation in form of a room in a home provided by Host Families. The young people and Host Families will be supported by Transitions staff.

Therapeutic
Delivering an emotional/ psychological/physical/spiritual
programme incorporating some of the leading innovative
psychological approaches to treatment.
Community Focused
Doing life together with house parents, professional workers and volunteers. Participating in and reconnecting with the church and wider community including partner providers, as well as family (where possible and appropriate), as part of the process of reintegrating and taking
their place in the wider world.
*Not to be confused with the TC “Therapeutic Community” model.
